Notice of privacy practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Effective 9 September 2026. It applies to Blooming Consultants, P.L.L.C., 4093 M-65, Hale, MI 48739. A paper copy is given to you at your first appointment and you can ask for one at any time by calling (989) 728-2364.

Our duties

We are required by law to maintain the privacy of health information that identifies you, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We are also required to notify you if a breach occurs that may have compromised the privacy or security of your information.

What this covers

Information that identifies you and relates to your mental health, the care you receive here, or payment for that care. It includes what you say in a session, appointment records, and what you send through the request a call form on this site.

How your information is used

Treatment
To provide your care and, with your written permission, to work alongside other people involved in it, such as a school or a medical team.
Payment
To bill your insurance and to obtain any authorization your plan requires. What an insurer is told is limited to what it needs to process the claim.
Running the practice
Scheduling, records, quality of care, and meeting legal obligations.

When information can be shared without your permission

These are narrow and they are set by law rather than by this practice.

  • Suspected abuse or neglect of a child or a vulnerable adult. Michigan law requires licensed counselors to report it.
  • A serious and imminent risk of harm to you or to somebody else.
  • A valid court order or subpoena, or another disclosure specifically required by law.

Outside those, information about you is shared only with your written permission, and that permission can be withdrawn.

Your rights

  • To see and get a copy of your record, with narrow exceptions.
  • To ask for a correction if something in it is wrong.
  • To ask for an accounting of certain disclosures.
  • To ask that we contact you in a particular way, or at a particular number.
  • To ask for restrictions on how your information is used and shared.
  • To get a paper copy of the full notice.
  • To complain, without it affecting your care.

How to raise a concern

Start by calling the practice on (989) 728-2364, or writing to Blooming Consultants, P.L.L.C., 4093 M-65, Hale, MI 48739. You can also complain to the United States Department of Health and Human Services Office for Civil Rights. Nobody will be penalized for making a complaint.

Changes to this notice

We can change this notice, and a changed notice will apply to information we already hold as well as to anything received afterwards. The current version is always the one posted on this page and the one available in paper at the office, and the effective date at the top says which version you are reading.

What this website does

The request a call form encrypts your message before it is stored, and the email that tells the practice an enquiry has arrived contains no detail at all: not your name, not your number, and not what you wrote. There is no analytics tag, no advertising pixel, no tag manager and nothing that follows you from one page to the next. The one request this site makes to another company is the map on the contact page, which loads from Google when you scroll to it; Google then receives your IP address and the fact that a map of this office was shown, and is not told which page you were reading. The website privacy policy sets that out in full.

Please do not send details about a health concern by email. Email is the least controlled channel in this system and it is deliberately kept out of the path.
