Paperwork you may need before an appointment, and a few places worth knowing about. Everything here is either a document you can download or a link to somebody else's website.
What is on this page
There is nothing on this page yet. The intake paperwork and the handouts we use most often are being put together, and they will appear here as they are ready.
Until then, call the office on (989) 728-2364 and ask. We are open Monday to Friday 9am to 5pm, and somebody will tell you what to bring to a first appointment.
Sending something back to us
This website does not accept anything you send it. There is no place to upload a filled in form here, and that is on purpose: a form you have written your name and your reasons on is health information, and email and web forms are not a safe way to move it.
